So the child s name remains with her last name on her social security card (which does not match the new birth certificate) and she is aware of this fact.

However, now the mother is saying its too big of a step to change the child s name, and she isn t ready.

The mothers maiden name was completely removed from the child s on the new certificate.

The mother willingly signed the petition to have the childs name changed, and my husband currently has a copy of the new birth certificate with his last name as the child s name.

When child support and visitation was established, the court also established the child to have her name changed to his last name, because the mother had previously given the baby her own last name.
